Two (June or July) utility bills must be presented to
the Prairie Vale office before your child’s enrollment
is complete. This is required for all students (New and
returning).
Please print out the forms, fill them out and return them to the school.
After completing and printing the on-line enrollment
forms please bring them to the school along with your student's original birth
certificate and shot records for us to make a copy.
These enrollment forms should only be filled out for new students.
You may also visit the school to fill
out your child's
forms.
Please download and read the
district's Network Agreement before
completing the enrollment packet. We did not include this in the forms in order
to cut down on the number of pages printed.
